How to obtain a Death certificate ?

Ahmedabad city (Gujarat):

    Name, address of the person requesting the death certificate with current date.
    Name of the deceased, place where death occurred & complete permanent residential address.
    Name of the father or husband/wife or son/daughter or the closest relative of the deceased.
    Sex, age and marital status of the deceased.
    Religion and nationality of the deceased.
    Cause of death, and doctor's certificate yes/no.
    Relationship with the deceased for the person requesting the death certificate.
    Details of any medical treatment received just prior to the death, if available.
    Signature of the person requesting the death certificate.

Baroda city (Gujarat): The birth and death registration department has been computerised, birth and death registrations will be done at all 12 ward offices. Send completed application form to the nearest ward office or the Civic center at Vivekanand Art Gallery, Badamdi Baug, Palace Road, Vadodara (Baroda), Gujarat. Offices are open from 10:30 am to 2:00 pm & 2:30 pm to 6:10 pm from Monday to Friday. Please note that Doctor's original certificate is required with the completed application form.

Jamnagar Mahanagarpalika (Gujarat): The birth and death registration form is common. Please ignore the fields relating to birth while applying. Documentation requirement is similar to other cities in Gujarat. If the deceased does not have any close relatives, then the death certificate can be obtained by hiring a lawyer with valid documents. Lawyers stamp and signature is also required for such cases.

Chennai city (Tamilnadu):

For deaths prior to 1/1/1980, contact Assistance Health Officer (Vital statistics) at Public information Centre, Ripon Buildings,Corportion of Chennai, Chennai- 600003, Tamilnadu.

For deaths pertaining to post 1/1/80 period, contact the Zonal Assistance Health Officer or Death registrar at Respective Zonal offices.

Fee for obtaining the certificate is Rs. 20 and is issued within 7 working days of satisfying records. General documents as to the name, address and other details about the deceased is required. Person requesting the death certificate is required to provide the evidence of relationship with the deceased and complete address with nationality.
